We are seeking a highly skilled and experienced Adult Advanced Practitioner to join our team at University Hospitals of North Midlands NHS Trust. As a key member of our Urgent Care Response Team, you will play a vital role in delivering high-quality patient care within the community.
Key Responsibilities:- Provide expert clinical care to patients in their own homes, utilizing advanced assessment and critical thinking skills to diagnose and manage complex health conditions.
- Work collaboratively with multidisciplinary teams to coordinate seamless care pathways and ensure effective communication with patients, families, and healthcare professionals.
- Prescribe and review medication, utilizing evidence-based practice and national protocols to ensure optimal patient outcomes.
- Assess and manage patients with mental health needs, referring to appropriate services as necessary.
- Lead and manage staff performance, training, and supervision, including annual appraisals and ensuring accurate and complete documentation.
Requirements:
- Registered Healthcare Professional with an MSc in Advanced Clinical Practice or equivalent.
- Independent Non-Medical Prescriber with expertise in acute care.
- Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Strong leadership and management skills, with experience in staff supervision and training.
