Job Summary:

We are seeking an experienced professional to join our Portage team as a Portage Inclusion Practitioner. The successful candidate will have a strong background in working with families and pre-school children with special educational needs and disabilities (SEND).

Key Responsibilities:

  • Provide a highly professional educational home visiting service for families of pre-school children with SEND
  • Work closely with families to support their child's development and well-being
  • Develop and implement individualized plans to meet the needs of each child
  • Collaborate with other professionals to ensure a comprehensive support network for families

Requirements:

  • Experience working with families and pre-school children with SEND
  • Strong knowledge of child development in the early years and approaches to supporting vulnerable families
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
