We are seeking a highly skilled and dedicated Nurse Practitioner to join our established and friendly nursing team at Springfield Surgery. As an autonomous practitioner, you will be responsible for delivering high-quality patient care and working collaboratively with our multidisciplinary team to meet the needs of our patients.
Key Responsibilities- Deliver evidence-based practice for patients presenting with long-term conditions and provide nursing and preventative health care to the practice population.
- Work collaboratively with the Duty Team to manage and provide care for patients with acute/urgent needs.
- Proactively identify, diagnose, and formulate treatment plans for patients at risk of developing a long-term condition.
- Work with other healthcare professionals to monitor, manage, and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs.
- Review medication for therapeutic effectiveness, appropriate to patient needs, and in accordance with evidence-based practice and national and practice protocols.
- Support patients to adopt health promotion strategies that promote healthy living and apply principles of self-care.
- Implement and participate in vaccination and immunisation programmes for both adults and children.
- Support and manage health needs of women presenting for family planning and cervical cytology consultations and other women's health issues.
- Undertake telephone triage to assess patients and plan any treatment required, bringing patients in or referring to a Doctor where appropriate.
- Provide a mentoring role to colleagues when required.
- Meet the needs of patients presenting for opportunistic wound care.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations, including initiation of effective emergency care.
- Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures.
- Participate in the maintenance of quality governance systems and processes across the practice.
- Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice.
- Provide enthusiastic day-to-day support and mentoring for Health Care Assistants.
- Participate in or lead Practice Meetings, Primary Health Care Clinical Team meetings, and audit meetings.
- Apply infection-control measures within the practice according to local and national guidelines.
- Provide cover for colleagues as and when necessary.
- Triage and treat patients, making any necessary referrals to other members of the primary health care team.
- Independently prescribe to patients while following agreed protocols.
- Work with the local community to provide health promotion information.
- Maintain continuous registration with the NMC.
- Take part in the revalidation process when relevant.
- Comply with the code of professional conduct as laid down by the NMC.
- Maintain a full driving licence.
- Keep up to date with current trends and procedures.
- Attend in-service training and maintain a portfolio of professional development.
- Maintain the highest possible standard of nursing care in line with agreed quality standards.
- Maintain a professional image and observe confidentiality at all times.
- Report any accidents or untoward incidents involving staff, patients, or self and provide written reports as required.
- Participate in annual performance review.
