Job Summary
We are seeking an experienced and compassionate Senior Care Support Worker to join our community services team. The successful candidate will work closely with our community and hospital colleagues to provide high-quality palliative and end-of-life care to patients and their families in their own homes.
Key Responsibilities
- Provide high-quality palliative and end-of-life care to patients and their families in their own homes
- Work closely with our community and hospital colleagues to ensure seamless care delivery
- Develop and maintain effective relationships with patients, families, and carers
- Contribute to the development and implementation of care plans that meet the individual needs of patients
- Participate in ongoing training and professional development to enhance skills and knowledge
Person Specification
- Level 3 Diploma/NVQ Level 3 in Health and Social Care for adults or equivalent
- A minimum of 1 year's care experience with palliative/end-of-life patients (any setting)
- Excellent communication and interpersonal skills
- Ability to work autonomously and as part of a team
