Job Title: Qualified Social Worker Team Manager

Job Description:

We are seeking an experienced and qualified Social Worker to join our team as a Team Manager. As a Team Manager, you will be responsible for leading a team of five agency social workers and ensuring that they undertake reviews in care homes and in a person's own home in a timely manner.

Key Responsibilities:

  • Manage a team of five agency social workers
  • Allocate work, monitor progress, and assure assessments
  • Ensure continuing health care checklists are implemented
  • Cover a wide remit of reviews and reassessment across the Wirral
  • Support with various adult social care projects and at-risk care home and domiciliary providers
  • Collaborate with Wirral contracts & commissioning and quality improvement teams

Requirements:

  • Social Work England Registered
  • Enhanced DBS
  • Social Work Degree or equivalent
