Job Summary

An exciting opportunity has arisen for a skilled Healthcare Assistant to join our Community Nursing Team at Gloucestershire Health and Care NHS Foundation Trust.

Main Responsibilities

As a Healthcare Assistant, you will be responsible for delivering high-quality care to patients in their own homes, working closely with our Community Nursing Team to provide a range of clinical services.

You will be required to communicate effectively with patients, carers, and other healthcare professionals, and to work efficiently and effectively in a fast-paced environment.

Key duties will include:

  • Delivering essential nursing care in a flexible manner, in a variety of community settings
  • Communicating and liaising with all members of the Primary Health and Social Care Team and other professionals
  • Supporting the daily planning and operation of patient clinics
  • Undertaking specific clinical tasks, including administration of routine medications, monitoring blood pressure, and wound care
  • Providing reassurance and support to patients and carers
  • Referencing patients appropriately to other members of our Multi-Disciplinary Teams
  • Contributing to patient assessments and care plans
  • Completing accurate record keeping of patients' notes and computer systems
Person Specification

We are looking for a motivated and compassionate individual with:

  • Level 1/GCSE English and Maths (or equivalent Adult Literacy and Numeracy)
  • Minimum of 5 GCSEs (or equivalent) including English, Mathematics, and a Science or Health and Social Care-based subject
  • NVA Level 3 Care certificate
  • Experience of direct adult patient care in a Health/Social Care setting
  • Evidence of Team working abilities
  • Proven ability to work unsupervised
