Senior Practitioner

We are seeking a highly skilled and dedicated Practitioner to join our Learning Disability team at Lincolnshire County Council. As a key member of our team, you will play a vital role in supporting individuals with learning disabilities and their families.
Main Responsibilities- Manage a caseload of clients with complex care needs, providing support and guidance to ensure their well-being and safety.
- Review and update care plans, working closely with key workers, families, and other stakeholders to ensure the best possible outcomes.
- Complete Deprivation of Liberty (DOL) applications, liaising with legal services, healthcare professionals, and other agencies as required.
- Develop and maintain effective relationships with families, carers, and other professionals to ensure seamless support and care.
- Work collaboratively with the team to achieve shared goals and objectives, contributing to the development of best practice and service improvement.
We are looking for a qualified and experienced Practitioner who is passionate about delivering high-quality care and support. You will have a strong understanding of the needs of individuals with learning disabilities and their families, and be able to work effectively in a fast-paced environment.
- Hold a relevant qualification in Social Work or Nursing, with a strong understanding of the principles of care and support.
- Have experience of working with individuals with complex care needs, including those with learning disabilities.
- Be able to work autonomously, with a high degree of independence and initiative.
- Have excellent communication and interpersonal skills, with the ability to build strong relationships with families, carers, and other professionals.
- Be able to work effectively in a team, contributing to the development of best practice and service improvement.
