About the Role

We are seeking a skilled and compassionate Case Worker to join our Contact, Assessment and Resolution Team (CART) at Shared Services Partnership. As a Case Worker, you will play a vital role in providing support and guidance to individuals, helping them to maximize their independence and wellbeing.

Key Responsibilities
  • Respond to incoming enquiries by telephone, online referrals, and web chat, and determine when to escalate cases to the management team.
  • Conduct wellbeing checks using a strengths-based approach and assess needs that could be met through the provision of occupational therapy equipment services.
  • Provide signposting where appropriate, directing individuals to relevant teams or partner agencies.
  • Achieve performance targets on the number of calls, online referrals, and web chat enquiries completed, as well as the quality of responses.
Requirements
  • Confident telephone communication skills and the ability to 'type and talk'.
  • Flexibility and adaptability.
  • Strong organizational skills and ability to embrace digital technology to support work.
  • Awareness of the Care Act is a plus.
  • Commitment to demonstrating the organization's values and behaviors.
