Job Overview

We are seeking a highly motivated and enthusiastic individual to join our Ferndown District Nursing team as a Community Healthcare Assistant. This is a fantastic opportunity to make a difference in the lives of our patients and contribute to the delivery of high-quality care in the community.

Main Responsibilities
  • Provide clinical care to patients in their own homes, under the supervision and support of the team.
  • Work collaboratively with the District Nursing Team to deliver patient-centered care.
  • Develop and maintain strong relationships with patients, families, and carers to ensure seamless care coordination.
  • Participate in the development and implementation of care plans, ensuring that patient needs are met and exceeded.
  • Contribute to the maintenance of accurate and up-to-date patient records, adhering to confidentiality and data protection policies.
Requirements
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Good IT skills, with experience of using electronic patient records and other healthcare software.
  • Valid driving license and access to a car, with the ability to travel to patients' homes as required.
  • Enhanced Disclosure and Barring Service (DBS) check, with the ability to work with vulnerable adults and children.
