Job Description

Job Title: Complex Care Supervisor

Job Summary:

We are seeking a highly skilled and experienced Complex Care Supervisor to join our team at Continuity Of Care Services. As a key member of our care management team, you will be responsible for overseeing the delivery of high-quality care services to our clients across Kent.

Key Responsibilities:

  • Supervise and support a team of Complex Care Assistants in delivering care services to clients in their own homes.
  • Conduct regular wellbeing checks and assessments to ensure the needs of clients are met.
  • Build strong relationships with clients and their families to ensure their care needs are met and their expectations are exceeded.
  • Perform care duties, including personal care, medication administration, meal preparation, and light domestic tasks.
  • Report to the Care Management team on all aspects of client care, including any concerns or issues that may arise.
  • Update and review care plans to ensure they are tailored to the individual needs of each client.
  • Manage competency assessments, including medication sign-offs, to ensure staff are competent to perform their duties.
  • Conduct risk assessments and reviews to identify potential risks and take steps to mitigate them.
  • Ensure compliance with all relevant regulations and standards, including those related to health and safety, data protection, and confidentiality.
  • Participate in the on-call rota to provide 24/7 support to clients and staff.
  • Ensure that all staff have the necessary training and support to perform their duties effectively.
  • Collaborate with other departments, including Care Managers and Resources Admin, to ensure seamless delivery of care services.

Requirements:

  • Complex care experience, including experience of working with vulnerable adults and children.
  • NVQ level 3 Health and Social Care qualification or above.
  • Full UK driving licence and access to a car.
  • Strong communication and interpersonal skills.
  • Excellent organizational and time management skills.
  • Effective problem-solving and decision-making skills.
  • Computer literacy and proficiency in using care management software.

Desirable Qualifications and Experience:

  • Care Certificate.
  • PEG feeding.
  • Positive Behaviour Training.
  • Makaton Sign Language.
  • Acquired Brain Injury (ABI) care.
  • Stoma care.
  • Total Parenteral Nutrition (TPN).
  • Epilepsy management.
