Job Title: Community Health Nurse

We are seeking a highly skilled and motivated Community Health Nurse to join our team in Brecon and Sennybridge. As a Community Health Nurse, you will be responsible for delivering high-quality nursing care to patients in their own homes, working autonomously and as part of a team to provide a range of nursing services.

About the Role

This is an exciting opportunity to work in a rural setting, providing care to patients with a variety of needs, including palliative care, chronic disease management, and wound management. You will be part of a supportive and progressive team, committed to delivering excellent standards of care to all people in our community.

Key Responsibilities
  • Deliver high-quality nursing care to patients in their own homes, working autonomously and as part of a team.
  • Assess and manage patients with a range of needs, including palliative care, chronic disease management, and wound management.
  • Work collaboratively with other healthcare professionals to provide a seamless service to patients.
  • Participate in the development and implementation of care plans, ensuring that patients receive the best possible care.
  • Contribute to the education and training of other healthcare professionals, sharing your expertise and knowledge to improve patient outcomes.
Requirements
  • NMC registration as a nurse.
  • Experience of working in a community setting, preferably in a rural area.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to speak or learn Welsh to a satisfactory level.
