Position Overview

We are seeking a qualified Senior Stroke Care Practitioner to join our esteemed stroke care team at The Mid Yorkshire Teaching NHS Trust. This role is ideal for individuals with a robust background in stroke management.

Our stroke service encompasses a dedicated assessment hub for immediate patient admission, a specialized Hyper Acute stroke unit, and a comprehensive stroke ward. Patients requiring further rehabilitation are transitioned to our rehabilitation facility. Our service also accommodates patients from surrounding areas during their acute treatment phase.

As a Senior Stroke Care Practitioner, you will function as an independent and accountable professional, collaborating with nursing, medical, and allied health teams to evaluate patient needs, initiate diagnostic procedures, and implement holistic treatment plans based on evidence-based practices.

The successful candidate will demonstrate motivation and the ability to work autonomously while integrating within the broader healthcare team, adhering to established protocols and guidelines.

Key Responsibilities

Your primary responsibilities will include:

  • Conducting thorough assessments and providing treatment for patients exhibiting stroke-related symptoms.
  • Delivering initial clinical diagnoses and determining appropriate referrals to specialized services.
  • Administering immediate care for patients with critical conditions, including life-threatening emergencies.
  • Engaging in outpatient evaluations, including TIA clinics, and managing patient follow-ups.
  • Serving as a mentor and resource for junior staff, providing guidance and clinical supervision.
