In this role, you will provide essential support to young adults with disabilities, promoting their independence and ensuring their physical and mental well-being. Our commitment is to empower every individual we assist, enabling them to lead fulfilling lives.
Key Responsibilities:
- Conduct thorough assessments of residents' needs and ensure that care plans are effectively implemented and monitored, aligning with NMC guidelines and current legislation.
- Collaborate with both clinical and non-clinical teams, taking a leadership role when necessary, while maintaining high standards of accuracy in clinical documentation and record-keeping.
- Actively participate in the multidisciplinary team, contributing to the formulation of care plans and utilizing appropriate assessment tools.
- Administer medications in accordance with the Nursing and Midwifery Council's guidelines.
- Ensure that all clinical documentation is maintained to a high standard, with patient notes filed appropriately.
- Work autonomously with confidence to deliver specialized and safe services to patients.
- Uphold and contribute to the standards of professional care, ethics, policies, and procedures.
Qualifications and Skills Required:
- Current NMC Pin registration (RGN).
- Legal eligibility to work in the U.K.
- Compliance in PEG, Catheter, and diabetes management is essential.
- Understanding of physical and learning disabilities, patient deterioration, and Epilepsy, or a willingness to learn.
- Proficient in documentation, care planning, and medication administration.
- Familiarity with manual handling, DOLs, MCA, and dysphagia, along with strong organizational and leadership skills.
- Knowledge of Care Home procedures and policies.
