We are seeking a highly skilled and experienced Field Care Supervisor to join our dynamic Complex Care team in East Sussex. As a key member of our team, you will play a vital role in ensuring the delivery of exceptional care to our clients with complex health needs.
Key Responsibilities- Collaborate with the Nurse Manager to oversee the care of a small caseload of clients with diverse complex care needs.
- Induct and train Care Assistants and Support Workers to ensure they possess the necessary clinical skills to meet clients' needs.
- Maintain regular shifts within your caseload to uphold familiarity with each client's requirements and provide consistent and personalized care per their care plan.
- Work with the Coordination Team to maintain 100% coverage of clients' care packages and organize solutions for absence cover.
- Identify and communicate upcoming staffing gaps and engage in recruitment to ensure seamless care delivery.
- Mentor support workers, conduct observations, and identify training needs to ensure the highest standard of care.
- Monitor client satisfaction, escalate concerns, and produce monthly reports for effective service management.
- Act in the best interest of clients, promptly reporting safety and welfare concerns, in line with safeguarding policies and procedures.
- Hold a Level 2 Diploma in Health and Social Care.
- Commit to attaining a Level 3 Diploma in Health and Social Care within two years of joining (funded by Superior Healthcare).
- Previous experience working with people who have disabilities and complex health needs, such as those with tracheostomies, ventilators, gastrostomy tubes, and bowel management care.
