Position Overview

The Dwyfor Ganol district nursing team is seeking to appoint a motivated Band 4 Clinical Support Practitioner to enhance our team. This role necessitates the individual to be an adept communicator and to adhere to the established standards and statutory regulations while functioning within the defined scope of the position and requisite competencies.

Main Responsibilities

The successful candidate will collaborate as part of a designated team to deliver evidence-based, high-quality clinical care tailored to the specific needs of patients, including those with chronic conditions. Under the guidance and support of a Registered Nurse, the Clinical Support Practitioner will adhere to the Health Board's protocols, policies, procedures, clinical governance standards, and care plans.

The responsibilities of the Clinical Support Practitioner include:

  • Facilitating effective communication with patients, caregivers, the multidisciplinary team, and the broader health and social care community.
  • Practicing in alignment with Health Board standards and statutory regulations while operating within the defined scope of the role and assessed competencies.
  • Utilizing personal effectiveness and knowledge to contribute to the development and delivery of a quality service.
  • Actively participating as a member of the multidisciplinary healthcare team, supporting registered practitioners.
  • Managing a small, defined caseload of patients under the supervision of a registered nurse.

Applicants must possess the QCF Level 4 Qualification - Foundation Degree in Health Care Level 4 or be in the process of obtaining it.

Proficiency in Welsh is desirable for this position; applications from both Welsh and English speakers are encouraged.
