About the Role

We are seeking a skilled and dedicated Social Worker to join our team in the Dartford Community Team. As a Social Worker, you will play a vital role in providing high-quality care and support to adults with a range of care and support needs.

Key Responsibilities
  • Undertake Care Act Assessments, Mental Capacity Act (MCA) Assessments, Court of Protection applications, and s42 safeguarding enquiries.
  • Work with adults with varying levels of complexity, including those with mental health needs, learning disabilities, and physical disabilities.
  • Develop and implement personalized care plans that meet the individual's needs and goals.
  • Collaborate with other professionals, including healthcare professionals, to ensure comprehensive care and support.
  • Provide emotional support and guidance to individuals and their families, promoting independence and well-being.
About You

We welcome applications from experienced social workers and newly qualified practitioners who can demonstrate capability of practice in accordance with current professional standards, legislative and policy frameworks.

You will hold a registration with a professional body (Social Work England) and have a high level of literacy and communication skills, both verbal and written.
