Clinical Pharmacist

About the Role

We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at Peel House Medical Practice. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the success of our practice.

Key Responsibilities

• Conduct structured medication reviews for patients with complex polypharmacy, implementing prescribing decisions and requesting appropriate tests.
• Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure that patients receive their medication post-discharge.
• Reconcile medicines from clinic letters with the support of the wider practice team.
• Support long-term condition reviews via face-to-face and telephone appointments.
• Contribute to reductions in medicine-related hospital admissions and re-admissions by supporting patients to get the best outcomes from their medicines.
• Identify and address medicine-related issues.
• Assist in managing patients and healthcare professional medicine queries.
• Increase patient quality and patient safety of prescribing through mechanisms such as audit and PDSA cycles, sharing findings with the wider team.
• Contribute to service improvement within enhanced services and QOF.
• Implement drug withdrawals and alerts, such as MHRA.
• Work with primary care professionals and patients to implement NICE and other evidence-based guidelines.
• Act as a source of medicine information for all practice team members and patients, providing guidance on supplies, doses, and side effects.

Person Specification

• Member of the Royal Pharmaceutical Society (RPS)
• UK-accredited MPharm degree or OSPAP qualification
• Clinical Pharmacy Diploma
• 52 weeks of pre-registration training in England, Scotland, or Wales, signed off as satisfactory by your tutor
• GPhC registration assessment with a pass result
• Responsible for own professional development and maintain competence within own clinical field
• Post-holder will also need to keep up with CPD requirements as specified by GPhC, completed or willing to enrol on the CCPE pathway
• Desirable: Hold an independent prescribing qualification

• Able to demonstrate practical experience of dealing with patients
• Evidence of Continuing Professional Development
• Experience of working in General Practice
• Awareness of a breadth of common acute and long-term conditions likely to be seen in primary care
