About the Role

We are seeking a dedicated and experienced Primary Care Mental Health Practitioner to join our multidisciplinary team in North Cumbria. This is an exciting opportunity to make a real difference in the lives of patients by providing responsive and person-centered mental health support within their local community.

Your Responsibilities

  • Provide specialist holistic assessments and support shared decision-making with patients, empowering them to actively participate in their care.
  • Deliver evidence-based brief biopsychosocial interventions tailored to individual needs, promoting recovery and well-being.
  • Act as a vital link between primary and secondary mental health services, facilitating seamless transitions and access to appropriate care.
  • Collaborate effectively with a wide range of professionals, including GPs, Practice Nurses, Social Prescribers, and voluntary sector organizations, to ensure comprehensive support for patients.
  • Demonstrate a strong understanding of health inequalities and implement strategies to promote accessibility and inclusivity in service delivery.

About You

You will be a registered mental health professional with a proven track record of clinical experience. Your expertise in psychological interventions, risk assessment, and safety planning will be essential in this role. A commitment to person-centered care and a passion for working within a collaborative team environment are highly valued.
