About the Role

We are seeking a highly skilled and experienced Care Coordinator to join our team at Invest Solutions Ltd. As a Care Coordinator, you will play a critical role in ensuring the delivery of high-quality care services to our clients.

Key Responsibilities
  • Manage and coordinate care worker rotas to ensure optimal coverage and client satisfaction.
  • Source and manage new care packages, building on existing relationships and identifying opportunities for growth.
  • Develop and implement operational strategies to improve the continuity of care and enhance the overall client experience.
  • Maintain accurate and up-to-date digital records for clients and care workers, utilizing our bespoke system.
  • Resolve customer queries in a timely and professional manner, ensuring a positive company reputation.
  • Ensure all operations are compliant with company policies and CQC regulations.
  • Contribute to the growth and service expansion targets set by the business.
Requirements
  • Education and previous experience in a care coordination role are essential.
  • Ideal NVQ/QCF level 3 in Health and Social Care or equivalent.
  • At least 2 years' experience working as a care coordinator for a Domiciliary Care Agency, with live-in care coordination experience preferred.
  • Previous experience of using Care-Planner and/or CM2000.
  • Excellent IT skills and knowledge of CQC Compliance.
  • People management or team leading experience.
  • Organized, methodical, and a good eye for detail.
  • Flexibility to participate in on-call duties when required.
