We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Pioneer Group. As a Social Prescribing Link Worker, you will play a vital role in supporting patients with social, emotional, and wellbeing needs.

Key Responsibilities
  • Support patients with complex cases, creating personalized support plans to enhance their overall wellbeing and quality of life.
  • Engage with patients through face-to-face appointments in GP surgeries and telephone consultations.
  • Build strong relationships with health and social care professionals to ensure seamless patient care.
  • Proactively manage a busy workload, prioritizing tasks and meeting deadlines.
Requirements
  • Confident self-starter with excellent communication and interpersonal skills.
  • Previous experience in a similar role, preferably in a community setting, particularly with vulnerable adults.
  • Proactive and organized, with the ability to manage multiple tasks and priorities.
