Job Title: Occupational Therapist - Reablement Specialist
Job Summary:
We are seeking two experienced Occupational Therapists to join our team at Tripod Partners. As a Reablement Specialist, you will play a crucial role in supporting adults of all ages to maintain their independence and live safely in their own homes.
Key Responsibilities:
- Conduct face-to-face reviews and assessments to identify individuals' needs and develop personalized plans to promote reablement and independence.
- Work collaboratively with clients, families, and other healthcare professionals to ensure seamless care coordination and effective service delivery.
- Triangulate referrals to determine the most suitable course of action, leveraging community resources whenever possible to minimize reliance on paid statutory services.
- Provide expert advice on equipment and adaptations to support clients' daily living needs.
- Develop and maintain strong relationships with clients, families, and community stakeholders to ensure ongoing support and advocacy.
Requirements:
- 2+ years of experience as an Occupational Therapist in a community care setting.
- Strong knowledge of reablement principles and practices.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid DBS clearance.
