Job Title: Adult Social Worker

We are seeking a highly skilled and experienced Social Worker to join our team at Connect2Halton. As a key member of our team, you will be responsible for providing expert social work services to individuals and families, assessing and managing complex situations, and making sound decisions to ensure the best possible outcomes.

Key Responsibilities:
  • Assess the need for social work services, identifying risk and determining the method of intervention to manage and minimize risk in complex situations.
  • Maintain and update accurate records of work undertaken, carrying out required administrative procedures and producing high-quality assessments and reports.
  • Prepare for and attend supervision sessions, staff meetings, and events, making use of available training and developmental opportunities to model good practice and set expectations for others.
  • Decide or advise on the use of appropriate social care and/or other services, resources, and contribute views on service provision to commissioners where applicable.
  • Comply with statutory obligations of the council and develop and provide training resources for social workers, support staff, and carers.
Requirements:
  • Qualified Social Worker with current registration with Social Work England.
  • Proven experience working within an Integrated Hospital Discharge Team, with the ability to hit the ground running.
  • Ability to manage a complex caseload and make sound decisions quickly and effectively.
  • Excellent communication, assessment, and organizational skills, with a commitment to collaborative working.
  • Knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and Safeguarding Adults procedures.
  • Proficiency in using IT systems, including knowledge of Eclipse (desirable).
  • Full UK driving license and access to a car for work purposes.
