About the Role

We are seeking a highly skilled and motivated Occupational Therapist to join our team at Central and North West London NHS Foundation Trust. As an Occupational Therapist, you will play a vital role in providing comprehensive assessment, advice, treatment, and discharge planning for patients across our Home First Therapy pathways.

Key Responsibilities

  • Provide occupational therapy assessments and deliver person-centered rehabilitation and individualized goals.
  • Undertake all aspects of clinical duties as an autonomous practitioner.
  • Work in the community pathways, assisting in admission avoidance assessments, the Getting People Home pathway to facilitate early supported discharge from the acute hospital, and inpatient rehabilitation pathways.
  • Communicate effectively and work collaboratively with all members of the multidisciplinary team and other services within Home First, as well as outside agencies.
  • Manage clinical risk within the Home First therapy caseload.
  • Be responsible for the safe and competent use of all appropriate equipment.
  • Work collaboratively with the MDT with discharge planning.
  • Comply and uphold standards of the HCPC and the Royal College of Occupational Therapists.
  • Be responsible for maintaining own competency to practice through CPD activities and to maintain a portfolio which reflects personal development.
  • Be responsible for attending and participating in formal supervision, including clinical supervision, in accordance with Trust policies and procedures and the standards of the RCOT.
  • Participate in the staff appraisal scheme as an appraisee and be responsible for complying with your agreed personal development programmes to meet knowledge and competency requirements.
  • Undertake the measurement and evaluation of your work and current practices through the use of evidence-based practice projects, audit, and outcome measures in conjunction with the Home First Lead Occupational Therapist.
  • Be an active member of the in-service training programme by attendance at, and participation in, in-service training programmes, tutorials, and individual training sessions.
  • Be responsible for planning your own caseload, prioritizing workload as situations arise.
  • Acquire an understanding of CNWL-MK clinical and HR policy and procedures.
  • Supervise designated team members and complete appraisals, maintaining accurate documentation.

Person Specification

  • Ability to drive and has access to a car for use at work.
  • HCPC registration.
  • Degree-level qualification in Occupational Therapy.
  • Experience post-registration in community/acute/stroke/rehabilitation settings.
  • Previous clinical experience of working in an NHS setting.
  • Evidence of recent continuing professional development.
  • Experience post-registration in stroke/rehabilitation settings.
  • Desirable: Experience of working in admission avoidance/supported discharge, experience of working in a multidisciplinary team, experience of supervising others, experience of working in frailty/inpatient settings.
