Adult Community Support Worker

We are seeking a highly motivated and enthusiastic Community Support Worker to join our team in Torbay and South Devon. As a key member of our specialist team, you will play a vital role in providing support to individuals living in their own homes, helping them to live with more independence and choice.
Key Responsibilities- Provide personal care and clinical duties as delegated, monitoring the patient's condition.
- Enable and/or assist individuals to remain in the community, promoting their safety and well-being in their own homes.
- Collaborate with other support workers and professionals to provide comprehensive care for service users.
- Provide good quality domiciliary support to service users, promoting independence and enhancing their quality of life.
- Assess the risk and needs of service users in accordance with the care plan.
Our service provides rapid short-term intervention up to a maximum of 7 days, aiming to prevent admission to hospital or residential/nursing home, and support discharges from hospital to facilitate earlier discharge. Our team operates on a 7-day rota system, including evenings, weekends, and Bank Holidays.
