Job Title: CPN - Older Adult CMHT

Hunter Gatherer Mental Health is seeking a skilled and dedicated Community Psychiatric Nurse (CPN) to join their Older Adult Community Mental Health Team (CMHT) in Greater Manchester.

Job Summary:

We are looking for a hardworking and diligent CPN to join our team, providing high-quality patient care to older adults in the community. The successful candidate will have a proven track record of working within a CPN recovery role and possess strong communication and interpersonal skills.

Responsibilities:

  • Provide mental health care and support to older adults in the community
  • Work collaboratively with the multidisciplinary team to achieve positive outcomes for patients
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Conduct regular assessments and reviews to monitor patient progress and adjust care plans as needed
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination

Requirements:

  • Current and valid registration with the NMC
  • Proven experience within a CPN recovery role
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Commitment to providing high-quality patient care
