Living Well Coach

The Living Well Coach will work within the Wellbeing Service to provide a coaching service for individuals with long-term health conditions and psycho-social needs. This role involves working closely with the wider Integrated Care Community (ICC) team to engage individuals in setting personalized goals and finding and accessing services and resources best suited to meet their needs and circumstances.
Key Responsibilities- Using coaching skills to support individuals identified by the GP practice and wider ICC, the Living Well Coach will work alongside people who want to improve their skills in self-management of their own health and wellbeing.
- The coach will listen to what is important to the person and help them to identify and commit to step-by-step activities, building towards their improvement goal.
- The post-holder will be expected to work in a range of environments, including visiting people in their own homes, in other GP practices, or community health/care settings.
