Adults Community Social Worker

We are seeking an experienced Adults Social Worker to join our Community Team. This team works with vulnerable adults within adult social services, providing support to those in need. The ideal candidate will have a Social Work qualification and a minimum of 1 year post-ASYE experience. They will have previous or current experience of working with Section 42, Care Act, and MCA assessments.

Key Responsibilities:
  • Develop and implement care plans for vulnerable adults
  • Work with multi-agencies to provide comprehensive support
  • Liaise with families to ensure the best possible outcomes for individuals
  • Work with adults with complex needs and/or experience in community settings
Requirements:
  • Social Work qualification (Degree/DipSW/CQSW)
  • Minimum 1 year post-ASYE experience
  • Previous or current experience of working with Section 42, Care Act, and MCA assessments
