Job Title: Community Health Nurse - Bradford

We are seeking a dedicated and compassionate Registered Nurse to join our Community Healthcare Team in Bradford. As a Community Health Nurse, you will provide high-quality nursing care to patients in their own homes or within community settings, focusing on promoting health, preventing illness, and managing long-term conditions.

Key Responsibilities:

Patient Care

  1. Provide holistic nursing care to patients in the community, including those with chronic illnesses, post-operative needs, and palliative care requirements.
  2. Perform clinical assessments, wound care, medication administration, catheter care, and other nursing procedures.
  3. Monitor and manage patients with complex health conditions, ensuring their care plans are followed and updated as necessary.
  4. Educate patients and their families on disease management, medication adherence, and healthy lifestyle choices.

Care Coordination

  1. Work closely with healthcare professionals, including GPs, social workers, physiotherapists, and occupational therapists, to coordinate patient care.
  2. Participate in multidisciplinary team meetings to discuss and plan patient care.
  3. Facilitate referrals to other healthcare services as needed.

Patient Education and Support

  1. Provide emotional and psychological support to patients and their families, addressing concerns and ensuring they feel supported in managing their health.
  2. Offer advice and education on managing long-term conditions, wound care, and prevention of complications.
  3. Empower patients to take an active role in their own care through education and self-management strategies.

Documentation and Reporting

  1. Maintain accurate and up-to-date patient records, including care plans, progress notes, and medication records.
  2. Ensure all documentation meets legal, ethical, and organizational standards.
  3. Report any changes in patient condition or concerns to the appropriate healthcare professionals promptly.

Health Promotion and Prevention

  1. Participate in health promotion activities within the community, such as health screenings and education sessions.
  2. Identify and address potential health risks in the community, promoting preventive measures.
Qualifications and Experience:

Essential

  1. Registered Nurse qualification with current registration with the Nursing and Midwifery Council (NMC).
  2. A minimum of 2 years of post-registration experience, preferably with experience in community nursing or a similar setting.
  3. Strong clinical skills, including wound care, medication administration, and the ability to manage complex care needs.
  4. Excellent communication and interpersonal skills, with the ability to build rapport with patients and families.
  5. Ability to work independently and manage a caseload effectively.
  6. A valid driver's license and access to a vehicle for home visits (if applicable).

Desirable

  1. Experience in palliative care, chronic disease management, or post-operative care.
  2. Additional qualifications or certifications in community health, chronic disease management, or related areas.
  3. Previous experience in a locum or temporary nursing role.
