Job Title: Care Coordinator

We are seeking a highly skilled and dedicated Care Coordinator to join our team at Pennine Care NHS Foundation Trust. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals with moderate to severe mental health needs.

Key Responsibilities:
  • Undertake comprehensive assessments to identify the needs of service users and develop personalized care plans.
  • Provide supervision and guidance to junior staff and students, ensuring they receive the necessary support and training to excel in their roles.
  • Work collaboratively with the multidisciplinary team to ensure seamless care coordination and effective communication.
  • Develop and maintain strong relationships with service users, carers, and other professionals to ensure a holistic approach to care.
  • Participate in risk assessments and formulation of risk management plans to ensure the safety and well-being of service users.
  • Contribute to the development of staff within the team, providing training and guidance to enhance their skills and knowledge.
Requirements:
  • Professional qualification in Social Work, Mental Health Nursing, or Occupational Therapy.
  • Registration with HCPC or NMC.
  • Experience in community mental health work, with a strong understanding of the Care Programme Approach.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality care.
