About the Role
We are seeking a highly motivated and experienced Clinical Practitioner to join our Dynamic Support Pathway team. As a Clinical Practitioner, you will work with adults with learning disabilities and/or autism, providing high-quality care and support to help them achieve their goals and improve their quality of life.
Key Responsibilities
- Work autonomously with a designated caseload of service users, taking responsibility for their care, quality, and clinical activity.
- Contribute to care quality performance and clinical activity by providing professional leadership and guidance, clinical advice, and management in relation to healthcare quality.
- Assess, plan, implement, and evaluate programs of evidence-based care for service users, taking into account changing healthcare needs and varying levels of complexity.
- Work collaboratively with a multi-disciplinary team to ensure effective care plans and treatments are in place for complex service users.
- Recognize, prioritize, and respond appropriately to urgent and emergency situations.
- Work collaboratively with other professionals, agencies, and stakeholders to ensure needs are met in relation to care input, support for ongoing care needs, and safe discharge.
- Maintain accurate, complete, and contemporaneous clinical records for each service user, in accordance with agreed policy.
- Participate in the development and implementation of new ways of working, implementing research-based practice to promote ongoing care improvement.
