The Coventry Community Neuro and Stroke Rehabilitation Services (CINSS) is seeking a driven and skilled Senior Clinical Practitioner to become a vital member of our multidisciplinary team.
About the Role:
This position offers a unique opportunity for a passionate Registered General Nurse (RGN) or Healthcare Professional (HCP) to contribute to a community-based therapy team focused on facilitating the safe discharge of patients from healthcare facilities.
Our patients require ongoing rehabilitation or management for neurological conditions, including both stroke and non-stroke related issues.
Key Responsibilities:
- Collaborate with acute services, rehabilitation facilities, and community networks to provide a patient-centered, goal-oriented rehabilitation approach.
- Play an essential role in the stroke care pathway, extending support into both stroke and inpatient rehabilitation environments.
- Enhance and refine your neurological and broader clinical skills, contributing to quality improvement initiatives and service advancements.
- Shape and develop your role within our established integrated therapy framework.
- Work closely with a diverse team of professionals, including Physiotherapists, Occupational Therapists, Psychologists, Speech and Language Therapists, and Therapy Assistants.
Essential Skills:
Strong communication skills and an inclusive team approach are crucial. You will be expected to work autonomously and efficiently within the Community Stroke and Integrated Neuro Rehabilitation Service, ensuring safe and effective patient management.
Responsibilities include:
- Conducting thorough medical assessments and history taking to establish clinical diagnoses.
- Formulating and implementing a range of medical and care interventions for patients with acute and chronic conditions.
- Engaging in interdisciplinary teamwork to support patients in achieving their rehabilitation goals.
- Participating in ongoing professional development through joint working and access to comprehensive training programs.
