Position Overview

Salary Range - £23,000-£25,000 Based on Experience.

Additional compensation for approximately 1 in 6 weekends on call and 1 evening per week.

Employment Type

This position is primarily full-time, comprising 35 hours weekly, though flexible hours may be considered for the right candidate.

Key Responsibilities

To guarantee that our clients receive the necessary assistance and a superior level of service.

  • Document client service inquiries following office protocols and return calls as needed.
  • Conduct next-day follow-up calls for new clients.
  • Execute 14-day Service Reviews, 3-month Quality Assessments, and 12-month Service Reviews, while assisting with 6-month Service Reviews and 9-month Quality Assessments, taking appropriate actions as required.
  • Develop, revise, and audit client care plans and modifications in digital care management systems.
  • Facilitate care consultations, service evaluations, and client support strategies.
  • Assist with risk assessments and care plans as necessary.
  • Maintain precise client and Care Professional records using Home Instead software and People Planner.
  • Aid in client and Care Professional introductions.
  • Perform observational competency assessments, supervisions, and support visits with Care Professionals.
  • Assist with client departure procedures, including funeral arrangements and sending sympathy cards.
  • Foster and sustain positive relationships with potential and existing clients, their families, and other professionals involved in their care.
  • Support the execution of self-audits and implement action plans with clients and Care Professionals.
  • Participate in daily office team meetings, team discussions, and communications with Care Professionals.
  • Provide coverage for care visits as needed to meet business requirements.
  • Update Access People Planner as necessary.
  • Monitor daily check-ins and medication alerts on Access Care Planning.
  • Engage in on-call responsibilities as required, including assisting with on-call handovers. Additional pay applies.
  • Undertake any other tasks deemed essential for the effective operation of the business.

Required Qualifications

Preferred Experience and Skills

Previous experience in a Health and Social Care environment, Care Coordination, or Logistics is preferred. Candidates must possess a valid driver's license due to rural service areas.

The role will often involve managing multiple requests, necessitating the ability to prioritize, multitask, and maintain composure in a dynamic office setting.

  • Experience in the care sector providing a diverse range of personal care services.
  • Familiarity with Medication Administration Record sheets and supervisory roles involving medication.
  • A strong commitment to delivering the highest quality care to clients, enabling them to live independently and happily in their own homes.
  • Understanding of relevant legislation and regulations pertaining to Health and Social Care.
  • Excellent communication skills with the ability to quickly establish rapport.
  • Proficiency in using care management technology, including providing support and training to Care Professionals.
  • Awareness of the importance of confidentiality in accordance with current legislation.
  • Competent in IT systems, with experience in Microsoft Office or Google Suite and virtual communication platforms, along with the ability to learn and adapt to new technologies and software as needed.
  • Must have a full driving license and reliable transportation if required to visit clients.
  • Organized and adaptable to meet the needs of the business.

An ideal candidate will possess exceptional time management and organizational skills, along with being self-motivated and dependable.
