POSITION OVERVIEW:


Oversee the delivery of care and assistance provided at an Extra Care Facility, ensuring that care/support personnel perform their responsibilities with the compassion necessary to deliver services that uphold the dignity, privacy, choice, independence, fulfillment, and rights of Service Users and their families.

Step in at other facilities during the absence of Senior Support Workers.

Key Responsibilities:
Maintain effective communication and foster strong working relationships with all staff and clients at the Extra Care Facility.

Collaborate with other housing, health, and social care professionals, as well as benefits, welfare, and advocacy organizations involved in the care and support of clients at the Extra Care Facility, as needed or instructed.

Adhere to all Care Support policies and procedures.

Notify the Extra Care Manager or Assistant Manager of any changes in the Service User's health or circumstances.

Assist in managing the staff schedule weekly, ensuring adequate staffing to meet the needs of all clients.

Participate in Needs Assessments, Risk Assessments, and Support Planning, identifying clients' needs, goals, and aspirations.

Engage in the On Call rota of the service.

When necessary, deliver effective, outcome-oriented reablement care and support.

Monitor and evaluate the progress of achieving outcomes and reablement goals with clients.

Participate in regular risk assessments, spot checks, staff evaluations, and performance reviews.

Ensure the implementation of an outcome-focused, reablement approach throughout the Extra Care facility, actualizing this in the daily provision of care and support services.


Support the Extra Care Manager and Assistant Managers in executing Housing Related Support tasks, such as advising on welfare benefits, providing guidance, promoting health, and encouraging independence.


Fully support a vibrant customer/residents committee at the Extra Care Facility, as this will serve as the foundation for organizing various activities and events.

Ensure a safe living environment for Service Users while respecting their choices and rights.

Contribute to safeguarding service users from abuse.

Ensure that Service Users are not placed at risk due to Care/Support activities.

Work collaboratively as part of a team while also being capable of working independently.

Attend supervision, training, and staff meetings as required.

Be available and willing to undertake any additional tasks specified by management relevant to the role and the needs of the Service Users.

Willingness to assist office staff with recruitment tasks, such as literacy assessments and interviewing.

Conduct audits of Care Support documentation, including MAR Charts, Log Sheets, and quality review records.
