Lead Practice Nurse

The Stephenson Medical Practice is seeking a highly skilled and experienced Lead Practice Nurse to join our primary care team. As a key member of our team, you will be responsible for leading and managing the nursing team, ensuring the delivery of high-quality patient care and services.
Key Responsibilities- Lead and manage the nursing team, ensuring effective communication and collaboration with other healthcare professionals.
- Develop and implement clinical policies and procedures, ensuring compliance with regulatory requirements and best practices.
- Provide clinical leadership and guidance to nursing staff, ensuring they have the necessary skills and knowledge to deliver high-quality patient care.
- Manage and coordinate the delivery of chronic disease management programs, including health promotion and disease prevention initiatives.
- Develop and maintain relationships with external healthcare providers and agencies, ensuring seamless care coordination and referrals.
- Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care and services.
- Collaborate with the practice management team to develop and implement business strategies, ensuring alignment with organizational goals and objectives.
- Registered Nurse (Nursing and Midwifery Council) qualification.
- Experience in leading and managing a nursing team in a primary care setting.
- Strong clinical knowledge and skills in chronic disease management, health promotion, and disease prevention.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work autonomously and as part of a team, with a strong commitment to delivering high-quality patient care and services.
