Neighbourhood Team Nurse

We are seeking an experienced Registered Clinician to join our Neighbourhood Team in Oxfordshire. As a Community Nurse, you will work closely with colleagues to provide high-quality, person-centered care to patients in their own homes.
Key Responsibilities- Undertake home visits to assess patients' needs and provide timely, high-quality care.
- Work collaboratively with GPs, hospital clinicians, and other agencies to prevent unnecessary hospital admissions.
- Develop and implement care plans to support patients with chronic conditions and complex needs.
- Provide education, support, and advice to patients and their carers to promote self-management and independence.
- Maintain accurate patient records and participate in clinical audit to ensure high standards of care.
- Current registration with the NMC.
- Two years post-qualification experience in a community setting.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- Proficiency in the use of Windows-based software, including Word and Excel.
