We are seeking a highly skilled and dedicated Health Visitor to join our team at Kent Community Health NHS Foundation Trust. As a Health Visitor, you will play a vital role in supporting the delivery of the Healthy Child Programme, working closely with families and communities to promote health and wellbeing.
Key Responsibilities- Conduct home visits to assess the health and wellbeing of children and families, providing guidance and support as needed.
- Develop and implement care plans to address the needs of children and families, working in partnership with other agencies and professionals.
- Provide education and support to families on healthy lifestyles, parenting, and child development.
- Work with other healthcare professionals to ensure seamless care and support for children and families.
- Contribute to the development and implementation of health promotion initiatives and campaigns.
To be successful in this role, you will need:
- Registration with the Nursing and Midwifery Council (NMC) as a Health Visitor.
- Experience of working with children and families in a healthcare setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of health promotion principles and practices.
