This is a challenging and rewarding role that will involve providing face-to-face support to children and their families, creating personalized care plans, and training and supporting staff with their clinical skills and activities.

**Key Responsibilities:**

* Design and deliver tailored care plans specific to individual client needs
* Effectively manage the multidisciplinary team through strong communication and teamwork
* Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs
* Deliver nursing interventions, including the management of medicine delivery
* Build and maintain sophisticated relationships with clients' families
* Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care

**Benefits:**

* Up to \u00a345,000 per annum DOE
* 2 days working from home per week / 3 days in the community
* Monday to Friday (9-5) hours
