Adult Community Care Specialist

We are seeking a skilled and compassionate Adult Community Care Officer to join our team in Ashford. As a key member of our Community Team, you will play a vital role in enhancing our locality working practice and putting the person at the heart of our care.
Key Responsibilities- Undertake initial enquiries and build relationships with individuals, their carers, and wider partners to support the management of crisis intervention.
- Conduct Care Act assessments and Mental Capacity Act assessments to ensure individuals receive the necessary care and support.
- Work collaboratively with communities to empower individuals to find trusted help and support locally from a range of sources.
- Develop and maintain effective partnerships with people and our wider partners to identify services or resources that can support well-being.
You will have experience of working with people with social care needs and undertaking initial assessments. A willingness to undertake a NVQ in Care Level 2 and 3 alongside Mathematics and English is essential.
