About the Role

We are seeking a highly skilled and compassionate Registered Children's Nurse to join our team as a Community Care Specialist. As a key member of our Children's Community Nursing service, you will play a vital role in providing high-quality care and support to children and young people with complex and palliative care needs.

Main Responsibilities
  • Coordinate and provide community-based care to children and young people with complex needs, working closely with families and other healthcare professionals.
  • Contribute to Children's Continuing Care Assessments, Safeguarding meetings, teaching, competency training, staff support, and team management.
  • Participate in palliative and end-of-life care, including contribution to a 24-hour on-call rota, and provide support to families during this challenging time.
  • Develop and maintain strong relationships with families, carers, and other healthcare professionals to ensure seamless care coordination.
  • Collaborate with the Solihull Children's Community Learning Disability Nursing Team and Solihull Special School Nursing Team to provide comprehensive care.
  • Contribute to the development and implementation of care plans, ensuring that they are tailored to the individual needs of each child.
  • Provide education and training to junior colleagues, promoting best practice and ensuring that they are equipped to provide high-quality care.
Requirements
  • Registered Children's Nurse or Registered Nurse Child Branch.
  • Minimum of 2 years post-registration community/complex needs experience.
  • Excellent communication and interpersonal skills, with the ability to work effectively with families, carers, and other healthcare professionals.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage a caseload.
  • Ability to work flexibly, including evenings and weekends, to meet the needs of the service.
  • Full current UK driving licence and access to a car for community visits.
