About the Role

We are seeking a skilled and compassionate Nurse Coordinator to join our team at NHS Suffolk and North East Essex ICB. As a key member of our healthcare team, you will play a vital role in coordinating and delivering high-quality care to our patients.

Key Responsibilities

  • Conduct comprehensive assessments and make eligibility recommendations for NHS Continuing Healthcare and NHS-funded Nursing Care.
  • Collate data and evidence from assessments to form reports and ensure that care and support plans meet individual needs.
  • Support individuals in accessing Personal Health Budgets and managing their care and support.
  • Carry out reviews of care and support and eligibility for NHS funding where required.
  • Work closely with local authority colleagues, care providers, and voluntary organizations to improve patient experience.

Requirements

  • Registered Nurse (Adult, Mental Health or Learning Disability) with active NMC Registration.
  • Excellent written and verbal communication skills, including providing and receiving complex, sensitive, or contentious information to a wide range of stakeholders.
  • Ability to work under pressure and meet deadlines, with a high level of professionalism and productivity.
  • Good IT skills, including Microsoft Word and experience in using a clinical system for recording patient information.
