We are dedicated to providing personalized and family-centered care, ensuring that individuals receive timely support within their communities.
As a valued Peer Support Specialist, you will be instrumental in delivering peer support and practical assistance to service users, aiding them in regaining control over their lives. By drawing upon your own lived experiences, you will provide relational support to Autistic adults, acting as a role model, guiding them to community resources, and fostering meaningful connections.
Through empathy and a profound understanding of their challenges, you will inspire hope and assist them in reaching their aspirations.
In this position, you will be uniquely positioned to facilitate and support information sharing that promotes choice, self-determination, and integration into local communities.
Key Responsibilities
Provide peer support to Autistic service users (Adults), assisting them in relation to their care plans and objectives on a one-on-one or group basis, focusing on building relationships and opportunities for mutual learning. Bring empathy and understanding into all interactions by sharing lived experiences and fostering hope. Maintain equality with and work alongside others facing similar experiences and challenges in their health and wellbeing, with a central focus on building trusting relationships rather than intervention. Positively support service users by connecting them with community resources and aiding them in achieving social inclusion. Promote understanding of the principles and practices of the Autism Outreach Lived Experience role (relationships and support). Collaborate with a team of professionals and service user families and carers in delivering peer-based approaches (informal support for individuals with similar life experiences). Advocate for individuals' needs, effectively communicating within the team and serving as a voice for service users. Ensure the service is responsive to the needs of Autistic adults with a trauma-informed approach. Maintain excellent communication and collaboration with colleagues, acting as an agent for systemic change and a champion of peer support with lived experience within and outside HCRG Care Group.About HCRG Care Group
