We are seeking a Support Worker to join our Columbia Drive service, which supports 4 Young Adults moving from residential college to their first home in the local community.
As a Support Worker, you will be at the forefront of providing high-quality, enabling care, helping individuals to overcome obstacles and live the life they choose.
Key responsibilities include:
- Implementing person-centred support plans and promoting independence
- Developing flexible, engaging activity plans that promote autonomy and personal growth
