Key ResponsibilitiesIn this role, you will provide essential support to service users residing in supported accommodation across the region. Your responsibilities will include:
- Collaborating with housing support professionals and facilitating access to primary healthcare services.
- Contributing to the achievement of targets set by Forward Trust and local commissioners.
- Conducting assessments (initial, comprehensive, and risk assessments) to inform treatment pathways.
- Partnering with various agencies, including training, employment, and family support services, to ensure comprehensive client care.
Your role will involve:
- Coordinating with healthcare providers to monitor and manage client health, minimizing potential risks.
- Encouraging clients to engage with the support services outlined in their care plans and risk assessments.
- Assisting clients with their care plans, including referrals to community support services, and providing guidance on housing, benefits, and employment training.
While the following skills and experiences are desirable, they are not mandatory as we provide a comprehensive induction and ongoing training program to enhance our workforce:
- Experience in substance misuse services.
- Proficiency in conducting thorough assessments, risk management plans, and developing SMART care plans.
- Understanding of holistic care needs and continuity of care within community services.
- Experience in promoting and supporting equality and diversity.
