As a Supportive Accommodation Facilitator within the Rough Sleeping Drug and Alcohol Team, you will play a vital role in assisting individuals facing challenges related to homelessness and substance misuse.

Key Responsibilities

In this role, you will provide essential support to service users residing in supported accommodation across the region. Your responsibilities will include:

  • Collaborating with housing support professionals and facilitating access to primary healthcare services.
  • Contributing to the achievement of targets set by Forward Trust and local commissioners.
  • Conducting assessments (initial, comprehensive, and risk assessments) to inform treatment pathways.
  • Partnering with various agencies, including training, employment, and family support services, to ensure comprehensive client care.
Client Engagement

Your role will involve:

  • Coordinating with healthcare providers to monitor and manage client health, minimizing potential risks.
  • Encouraging clients to engage with the support services outlined in their care plans and risk assessments.
  • Assisting clients with their care plans, including referrals to community support services, and providing guidance on housing, benefits, and employment training.
