Position Overview

The Rehabilitation Support Assistant plays a vital role in providing compassionate and high-quality care to patients undergoing rehabilitation. This position involves assessing, implementing, and evaluating the care needs of a designated group of patients under the indirect supervision of qualified nursing or therapy staff, both in hospital and community environments.

The successful candidate will rotate through various areas of the rehabilitation service.

Key Responsibilities

  • Deliver a compassionate rehabilitation service that recognizes and addresses the unique needs of each patient.
  • Manage and prioritize a personal caseload of patients following initial assessments conducted by registered professionals.
  • Follow established assessment protocols and conduct falls assessments as directed by supervising staff.
  • Implement and advance individualized treatment plans, addressing social, physical, psychological, and cognitive challenges while providing relevant updates to supervising professionals.
  • Encourage and support patients in achieving their highest level of independence through functional and therapeutic activities, ensuring their dignity and privacy are maintained.
  • Monitor the well-being of patients in rehabilitation, promptly reporting any changes in their condition or behavior to the appropriate professionals.
  • Educate patients on the risks associated with falls and the rehabilitation process.
  • Assess patients' mobility, instruct on the proper use of mobility aids, and supervise their rehabilitation progress.
  • Provide instruction on exercises aimed at improving balance, coordination, and overall function.
  • Teach correct body positioning and posture techniques.
  • Evaluate patients' abilities to transfer independently and discuss options with supervising staff.

Position Requirements

The successful candidate will demonstrate leadership qualities and a commitment to innovative practices within the rehabilitation service, alongside a dedication to lifelong learning.

Responsibilities include:

  • Managing a diverse clinical caseload as part of the Acute Response and Rehabilitation in the Community and Hospital Services.
  • Providing essential occupational therapy, nursing, physiotherapy, and discharge interventions to help patients achieve optimal independence and quality of life.
  • Identifying complex discharge cases and facilitating assessments to ensure safe transitions to community services.
  • Escalating concerns to the Clinical Team Leader and Service Manager as necessary.
  • Contributing to multi-disciplinary team meetings to ensure patient needs are met and to promote advanced care planning.
  • Working collaboratively with patients to enhance their confidence and functional abilities following illness, injury, or disability.

Essential Qualifications

  • A formal qualification relevant to the role (degree/diploma/NVQ/BTEC or equivalent experience).
  • A minimum of two years' experience in a health or social care environment.
  • Experience working with older adults with chronic conditions.
  • A valid UK driving license and access to a vehicle for work purposes.
  • Understanding of the impacts of aging and frailty, as well as the needs of patients at risk of falls.

Desirable Qualifications

  • Basic computer skills.
