We are seeking a highly skilled and experienced Critical Care Transfer Practitioner to join our team at University Hospitals Bristol and Weston NHS Foundation Trust.
About the RoleThis is an exciting opportunity to work in a dynamic and innovative environment, providing high-quality transfer care to critically unwell patients across the South West.
As a Critical Care Transfer Practitioner, you will be responsible for delivering specialized care to patients requiring transfer from one hospital to another, working closely with multidisciplinary teams to ensure seamless transitions.
Key Responsibilities- Provide high-quality transfer care to critically unwell patients, adhering to national guidelines and standards.
- Work collaboratively with multidisciplinary teams, including doctors, nurses, and other healthcare professionals, to ensure effective patient care.
- Develop and maintain strong relationships with referring centers, ensuring smooth communication and coordination of patient transfers.
- Support and mentor new team members, promoting a culture of excellence and continuous learning.
- Contribute to service projects, audit, and research, driving quality improvement and innovation within the Critical Care Transfer Service.
