Job Title: Registered Nurse - Community

We are seeking a highly skilled and dedicated Registered Nurse to join our Community Nursing team in the Exmouth, Budleigh, and Woodbury Cluster. As a Registered Nurse in this role, you will provide person-centered care to patients in their own homes or alternative residential settings, promoting independence and optimizing quality of life.

Key Responsibilities:
  • Assess, plan, implement, and evaluate nursing care for patients in the community
  • Provide education and support to patients and their families to manage long-term conditions and prevent hospital admission
  • Work collaboratively with a multidisciplinary team to meet the nursing needs of individuals in their home setting or alternative residential settings
  • Develop and maintain effective relationships with patients, families, and healthcare professionals to ensure seamless care delivery
  • Participate in education and training opportunities to enhance skills and knowledge in community nursing
Requirements:
  • Registered Nurse qualification and current registration with the NMC
  • Experience in community nursing or a willingness to learn and develop skills in this area
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
