Position Overview

As a Lead Community Nurse within our esteemed Health Board, you will be tasked with providing exemplary leadership for nursing teams operating in community settings. A wealth of relevant clinical experience at a senior level is essential, along with a comprehensive understanding of community healthcare environments. Your role will involve demonstrating robust clinical and professional leadership within the Primary Care & Communities Care Group, serving as a visible and supportive role model for your colleagues.

To excel in this position, you must exhibit a profound understanding of patient management processes, including patient flow, quality assurance, and safeguarding protocols. Exceptional communication skills are crucial, as you will be expected to work autonomously, offering specialized professional guidance to ensure the delivery of high-quality care. This includes the interpretation and application of national policies and the support of systems for professional revalidation and performance management of nursing staff accountable for delivering safe and effective care.

As a vital member of the Senior Nursing team, you will collaborate closely with the Lead Nurse and Head of Nursing to guarantee the provision of high-quality, safe, and patient-centered care.

Key Responsibilities

The successful candidate will be responsible for:

  • Providing professional leadership, management, and governance for community nursing services.
  • Guiding the development and direction of registered nursing and allied healthcare professionals.
  • Supporting the Senior Nursing Team and Service Manager to ensure the highest standards of nursing care are delivered, monitored, and evaluated.
  • Interpreting national and health board policies and advising on their implementation within your area of responsibility.
  • Overseeing processes and audits to assess the quality of care, clinical practices, incidents, and professional registrations.
  • Conducting investigations into clinical incidents, complaints, and professional matters in accordance with Health Board policies and acting on findings and recommendations.
  • Participating in a Senior Manager on-call rota, being available for on-call duties as per the established process.
  • Ensuring that your teams consistently strive for improvements in patient services, aligning with the principles of Values Based Healthcare and fostering a culture of continuous enhancement.
