About the Role

Norfolk Primary Care C.I.C. is seeking a highly skilled and motivated Mental Health Coach to join their multidisciplinary team within the Primary Care Network.

Key Responsibilities
  • Work in a patient-facing role to provide personalized support to individuals, their families, and carers to empower them to manage their own mental health and live independently.
  • Provide assistance and advice to GPs and other members of the primary healthcare team in managing the wellbeing of the practice population.
  • Engage patients in coaching and motivate them through multiple sessions to identify their needs, set goals, and support them to achieve their personalized mental health objectives.
  • Provide interventions such as self-management education and peer support.
  • Support personal choice and positive risk-taking while ensuring that patients understand the accountability of their own actions and decisions.
  • Work in partnership with other social prescribing services to connect patients to community-based activities or support charities that enable them to take increased control of their mental health and wellbeing.
