About the Role

We are seeking a highly skilled and dedicated Community Staff Nurse to join our team at Wrightington Wigan & Leigh Teaching Hospitals NHS Foundation Trust. As a Community Staff Nurse, you will play a vital role in providing high-quality care to patients in our Frailty SDEC service.

Key Responsibilities
  • Provide exceptional nursing care to patients in our Frailty SDEC service, ensuring their physical, emotional, and social needs are met.
  • Work collaboratively with the multidisciplinary team to develop and implement individualized care plans that promote patient independence and well-being.
  • Conduct thorough assessments of patients' needs, identifying areas for improvement and implementing evidence-based interventions to enhance their care.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination and transitions.
  • Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that promote best practice and patient safety.
  • Collaborate with senior nursing staff and specialist nurses to set and monitor standards of care, ensuring that patients receive the highest level of care.
  • Assist with the investigation and resolution of complaints and concerns, providing a high level of customer service and ensuring that patients' rights are respected.
Requirements
  • NMC registration and a valid PIN.
  • Experience in a relevant speciality, with a strong understanding of frailty and rehabilitation care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Ability to maintain and improve standards of care, with a focus on patient-centered care and safety.
  • Flexibility and a positive approach to work, with a willingness to adapt to changing priorities and demands.
