About the Role

We are seeking a highly skilled and compassionate Substance Misuse Nurse to join our team at Change Grow Live. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals struggling with substance misuse.

Key Responsibilities
  • Conduct comprehensive physical and mental health assessments to identify individual needs and develop personalized care plans.
  • Provide education, health promotion, and screening for blood-borne viruses (BBVs) and offer Hepatitis A and B vaccinations as necessary.
  • Work collaboratively with individuals, families, and other healthcare professionals to develop and implement effective treatment plans.
  • Contribute to the development and implementation of harm reduction strategies and provide support to individuals in crisis.
  • Participate in regular team meetings and contribute to the development of service policies and procedures.
Requirements
  • Registered Nurse with a current PIN and a clear understanding of harm reduction and health promotion interventions.
  • Ability to assess and co-produce recovery plans with individuals and formulate written reports as necessary.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Commitment to facilitating positive outcomes for individuals and promoting the values and vision of Change Grow Live.
  • Ability to travel between services and work flexibly to meet the needs of the role.
